Who Provides Your Care

Medical care through the GlydeRX Platform is provided by OpenLoop Healthcare Partners, PC and its affiliated professional corporations (including OpenLoop Healthcare Partners California, PC; and Untitled Health PLLC), which operate as a HIPAA Affiliated Covered Entity ("ACE"). These entities may share your protected health information (PHI) with each other as needed to carry out treatment, payment, and healthcare operations. A single Privacy Officer is responsible for HIPAA compliance across all ACE participants.

GlydeRX, LLC is a business associate of these covered entities and handles PHI pursuant to a Business Associate Agreement, consistent with HIPAA requirements.
